Module 1: Orientation, Role, Person-Centered Care & Client Rights

What is Person-Centered Care?

Person-centered care places the individual at the heart of their own care plan. As a Home Health Aide, you will work with clients to understand their preferences, values, and goals, ensuring care respects their dignity and autonomy.

Role of a Home Health Aide

Home Health Aides provide personal care assistance to clients in their own homes. Your responsibilities include activities of daily living (ADL) support, companionship, light housekeeping, and reporting changes to the supervising nurse.

Key Responsibilities:
  • Assist with bathing, grooming, and dressing
  • Help with toileting and incontinence care
  • Provide mobility assistance and transfers
  • Perform light housekeeping and meal preparation

Client Rights

Clients have fundamental rights to dignity, privacy, autonomy, and quality care. You must respect these rights at all times and report any violations to your supervisor immediately.

Module 2: Aging and Recognizing Changes in Older Adults

Normal Changes with Aging

As people age, various physical and cognitive changes occur naturally. Understanding these changes helps you provide appropriate care and recognize when changes may indicate illness or concern.

Common Age-Related Changes:
  • Vision: Presbyopia, cataracts, reduced adaptation to light changes
