Licensed Practical Nurses

The Licensed Practical Nurse (LPN) provides skilled nursing care to clients in their homes while also supporting and supervising Direct Support Professionals (DSPs). This role ensures high-quality care delivery, proper execution of care plans, and oversight of day-to-day client needs.

Key Responsibilities:

Skilled Nursing Care

• Provide in-home nursing services within LPN scope of practice
• Perform assessments and monitor client condition
• Administer medications and treatments as ordered
• Provide wound care and other clinical interventions as needed
• Educate clients and caregivers on care plans and health needs

DSP Oversight & Support

• Supervise DSPs providing personal care services
• Train and guide DSPs on proper caregiving techniques (ADLs, safety, infection control)
• Conduct field visits to ensure quality of care
• Address performance issues and reinforce standards

Care Coordination

• Ensure care plans are followed and updated as needed
• Communicate with families, providers, and internal team members
• Report changes in client condition promptly

---

Documentation & Compliance

• Complete accurate and timely nursing notes and visit documentation
• Review DSP documentation for completeness and accuracy
• Ensure compliance with agency policies and care standards

---

Quality & Safety

• Monitor infection control and safety practices in client homes
• Identify risks and implement corrective actions
• Report and document incidents appropriately

Qualifications:

• Active Licensed Practical Nurse (LPN) license (state-required)
• Clinical judgment and communication skills
• Organized and able to manage multiple clients/staff

Preferred:

• CPR/First Aid certification

Work Environment:

• Field-based role providing care in client homes

Physical Requirements:
